经阴道彩色多普勒血流显像对绒毛膜癌的诊断价值

Value of Transvaginal Color Doppler Flow Imaging in Diagnosing Chorionic Carcinoma

目的 探讨经阴道彩色多普勒 (CDFI)及脉冲多普勒 (PD)超声对绒毛膜癌的诊断 ,疗效观察及判断疾病转归的应用价值。方法 对 1 4例绒毛膜癌患者进行经阴道彩色多普勒及脉冲多普勒 (PD)超声检查 ,并记录病灶及子宫动脉血流阻力指数 ,同时记录血清绒毛膜促性腺激素 (β- HCG)值。结果 1 4例绒毛膜癌患者中 ,2例为附件区包块型 ,2例为阴道壁包块型 ,1例为子宫颈包块型 ,9例为子宫肌壁肿瘤型 ,不同类型的二维声像图特征不同 ,超声图像、病灶及子宫动脉血流阻力指数、血清绒毛膜促性腺激素值三者呈正相关关系。结论 经阴道彩色多普勒超声显示病灶异常血流图像和脉冲多普勒 (PD)检测病灶及子宫动脉低 RI值是诊断绒毛膜癌的重要指标 ,对监测化疗效果。

Objective To probe the applied value of transvaginal color Doppler flow imaging(CDFI)and pulsed Doppler (CPD) in diagnosing chorionic carinoma.Methods Fourteen cases with chorionic carcinoma underwent TVCDFI and PD.The focuses,RI of uterine artery and serum β HCG level were recorded.Results Among the 14 cases with chorionic carcinoma,2 cases belonged to the mass of appendage type;2 cases,the mass of vaginal wall type;1 cases,cervix type;9 cases as uterine wall tumor type;different types had different ultrasound characteristics.The ultrasound images,focuses,RI of uterine artery and serum β HCG value,these three,had positive proportional relationship.Conclusions CDFI can demonstrate the focal abnormal flow images and PD can detect the lower RI of the focal and uterine arteries which are important indicators for diagnosing chorionic carcinoma,for monitoring the effects of chemotherapy and selecting accurate treatment remedies.
