饮食干预对糖尿病高危人群生物学和血生化指标的影响 被引量:9

摘要 目的:研究饮食干预方式对糖尿病高危人群生物学和血生化指标的影响,探讨饮食治疗的有效性。方法:293例糖尿病高危患者(空腹血糖5.3~6.9mmol/L或口服75g葡萄糖2h后血糖7.8~11.0mmol/L),按有关条件排除后剩余病例自愿进入以下两组:①饮食干预组:91例,选用全国预防糖尿病协作组推荐的饮食干预方案,持续6个月。②对照组:98例,未参加锻炼和进行其他方式干预。饮食干预组治疗前后(对照组首次检查后6个月复查)进行生物学指标(体质量、体重指数、腰围、腰臀比、收缩压、舒张压、静息心率)和血生化指标(空腹血糖、葡萄糖负荷试验2h后血糖、糖化血红蛋白、血清总胆固醇、三酰甘油、高密度脂蛋白、低密度脂蛋白)检测。结果:两组患者首次检查生物学和血生化各项指标分布接近(P均>0.05)。饮食干预组患者治疗后的体质量(78.7±22.0)kg、体重指数(24.9±8.8)kg/m2、腰围(85.7±24.7)cm、腰臀比(0.87±0.05)、血糖(5.66±1.53)mmol/L、葡萄糖负荷2h后血糖(7.84±1.73)mmol/L、糖化血红蛋白(5.67±0.48)%、血清总胆固醇(5.59±0.93)mmol/L、高密度脂蛋白(1.39±0.39)mmol/L均较治疗前有显著改善(t=2.136~2.392,P均<0.05),但对照组6个月后上述各项结果均较首次检查无明显变化(P均>0.05)。 AIM:To study effect of dietary intevention on the biological and blood biochemical indice in high risk group with diabetes,to explore the effictiveness of diet therapy. METHODS: 293 high risk patients with diabetes (a fasting glucose of 5.3-6.9 mmol/L or glucose of 7.8-11.0 mmol/L 2 hours after 75 g oral glucose)were chosen,the remained patients were divided into two groups after excepting a number of them according to certain conditions:①The dietary intevention group(DIG):91 patients took the dietary intevention plan recommended by National Diabetes Prevention Cooperative Group for 6 months.②Control group:98 patients didn't take any exercises and other intervention.The tests of biological indices[body mass,body mass index(BMI),waistline,waist to hipratio(WHR),systolic pressure(SP),diastolic pressure(DP)and resting heart rate(RHR)] and blood biochemical indices [fasting glucose, glucose 2 h after glucose stress text, glycohemoglobin,serum total cholesterol, triacylglycerol , high density lipoprotein(HDL) and low density lipoprotein(LDL)] were detected in DIG before and after therapy(those in the control group were re exammed 6 months after the first examination). RESULTS:The biological and blood biochemical indice in both groups for the first test were close(P >0.05). Body mass (78.7±22.0)kg, BMI(24.9±8.8) kg/m2, waistline(85.7±24.7)cm, WHR(0.87±0.05), glucose(5.66±1.53) mmol/L, glucose 2h after glucose stress text(7.84±1.73)mmol/L, glycohemoglobin(5.67±0.48)mmol/L,serum total cholesterol(5.59±0.93 )mmol/L, HDL(1.39±0.39 )mmol/L in DIG significantly improved compared with the those of before therapy(t=2.136-2.393, P< 0.05), while those in control group 6 months later were not significantly different from those of the first examination (P >0.05).
