摘要
目的 了解中国北方人食管癌、贲门癌的易感性与细胞周期蛋白 (Cyclin)D1基因第 870位核苷酸A/G(A870G)多态性的关系。方法 应用PCR SSCP方法 ,检测了 12 0例食管癌、5 8例贲门癌及 12 2例健康对照组外周血白细胞CyclinD1(A870G)的基因多态性。结果 食管贲门癌组与对照组CyclinD1(A870G)A/G的等位基因分布差异无显著意义 (P =0 0 75 )。非吸烟的肿瘤患者CyclinD1的基因型分布与对照组比较差异无显著意义 (P >0 0 5 )。而吸烟并携带A/A基因型可使患食管贲门癌的危险性增加 2 5倍 ,其校正比值比为 2 5 7,95 %可信区间为 1 19~ 5 5 7。携带G/G基因可降低食管癌发病的风险 ,其校正比值比为 0 39,95 %可信区间为 0 18~ 0 82。贲门癌患者组携带A/A基因型的频率 (34 4 8% )高于对照组 (2 3 77% ) ,但差异无显著性 (χ2 =2 2 8,P =0 131)。结论 在中国北方人 ,吸烟并携带CyclinD1(A870G)A/A基因型是患食管贲门癌的危险因素 ,而CyclinD1G/G基因型对食管癌的发病可能具有防护作用。
Objective To investigate the association of Cyclin D1 (A870G) single nucleotide polymorphism (SNP) with the susceptibility to esophageal and cardiac cancer in northern Chinese population. Methods By polymerase chain reaction-single strand conformation polymorphism analysis (PCR-SSCP), the Cyclin D1 (A870G) genotyping was performed among 178 patients with esophageal or esophageal -gastric junction carcinoma (120 with esophageal squamous cell cancer and 58 with cardiac adenoma cancer) and 122 health controls. Results There were no significant differences in Cyclin D1 (A870G) allele frequencies between cancer patients and health controls(P=0.075). There is no genotype distribution difference was found between non-smoker patients and controls(P>0.05). The risk for esophageal and cardiac cancer is 2.5 times higher in A/A genotype carried smokers than that in A/G or G/G genotype carried non-smokers, with the adjusted Odd Ratio of 2.57 (95% confidence interval is 1.19~5.57). The G/G genotype reduces the susceptibility to esophageal cancer, with the adjusted Odd Ratio of 0.39 and 95% confidence interval of 0.18~0.82. The A/A frequency in cardiac patients(34.48%)is higher than that in health controls(23.77%)but the difference does not reach significant(P=0.212). Conclusion In northern Chinese population, the smoking individuals carrying Cyclin D1 (A870G) A/A genotype increase the susceptibility to esophageal and cardiac cancer. The G/G genotype probably plays a protecting role in the occurrence of esophageal cancer.
