摘要 目的 :探讨血小板衍生内皮细胞生长因子 (PD ECGF)在胃癌中表达的临床病理意义及其与肿瘤血管生成的关系。方法 :采用免疫组化S P法检测 5 8例胃癌 ,2 6例癌旁非瘤组织PD ECGF的表达 ,应用抗CD34 单抗标记血管内皮细胞测定肿瘤间质微血管密度 (MVD)。结果 :胃癌组织PD ECGF阳性表达率 (82 76 % )明显高于癌旁非瘤组织 (15 38% ,P <0 0 0 1)。PD ECGF阳性表达组的平均MVD值(30 6 2± 12 0 2 )明显高于PD ECGF阴性表达组 (2 0 15± 7 88,P <0 0 5 )。并且 ,随着PD ECGF表达程度的加强 ,MVD值增加。此外 ,PD ECGF表达与胃癌浸润深度、TNM分期密切相关。浸润达浆膜层的胃癌组织 ,PD ECGF阳性表达率 (94 4 4% )明显高于未累及浆膜层者 (6 3 6 4% ,P <0 0 1)。TNM分期为Ⅲ~Ⅳ期的胃癌组织 ,PD ECGF阳性表达率 (93 75 % )明显高于TNM分期为Ⅰ~Ⅱ期的胃癌组织(6 9 2 3% ,P <0 0 5 )。结论 :PD ECGF是胃癌的促血管生成因子之一 ,在促进肿瘤的浸润和发展起一定作用。 Objective To investigate the expression of platelet derived endothelial cell growth factor (PD ECGF) and its relation to angiogenesis of gastric carcinoma.Methods CD 34 was used to explore the microvessel density (MVD) as the marker of vascular endothelial cell.PD ECGF was detected with immunohistochemistry on 58 cases of gastric carcinoma tissue and 26 cases of adjacent non neoplastic tissue.Results Positive expression of PD ECGF in tumor tissue (82 76%) was stronger than that in corresponding adjacent non neoplastic tissue (15 38%, P <0 001).MVD was significantly higher in the PD ECGF positive expression group (30 62±12 02)than that in the PD ECGF negative expression group (20 15±7 88, P <0 05).Furthermore,MVD increased with the increase of the level of the PD ECGF expression.In addition,there was a close relationship between the expression degree of PD ECGF and the depth of tumor invasion and stage of TNM.The rate of PD ECGF expression in the tumors with serosa invasion was 94 44%(34/36),which was significantly higher, P <0 01,than that in those without serosa invasion,63 64%(14/22).The rate of PD ECGF expression of TNM stage Ⅲ-Ⅳ in gastric carcinoma (93 75%,30/32) was significantly higher ( P <0 05) than that of TNM stage Ⅰ-Ⅱ in gastric carcinoma (69 23%,18/26).Conclusion PD ECGF is one of angiogenic factors and might contribute to the tumor invasion as well as development in gastric carcinoma.
