摘要
目的了解尿转铁蛋白(TRF)在糖尿病肾病早期诊断中的价值。方法采用免疫散射速率法测定45例2型糖尿病患者及18例正常对照者24h尿转铁蛋白(TRF)、尿白蛋白(mA)含量并进行比较。结果糖尿病患者3组即正常蛋白尿组、微量白蛋白尿组和临床蛋白尿组24h尿转铁蛋白(TRF)检测结果分别为2.73±0.26mg/L、8.52±2.15mg/L和45.25±16.42mg/L,24h尿白蛋白(mA)结果分别为18.45±3.65mg/L、135.26±15.25mg/L和736.20±152.34mg/L,与对照组(24h尿TRF和mA分别为1.85±0.23mg/L和15.43±2.67mg/L)比较24h尿白蛋白(mA),转铁蛋白(TRF)均明显升高(P<0.01和P<0.001);3组间比较亦存在明显差异(P<0.001),TRF与MA呈显著正相关(r=0.832,P<0.001),且TRF在正常蛋白尿组与对照组之间亦存在这差异(P<0.001)。结论24h尿转铁蛋白(TRF)与24h尿白蛋白(mA)的一样,能反映糖尿病患者肾脏的早期改变,而且TRF比mA更敏感。
Objective To study the significance of urine transferrin in the early diagnosis of diabetic nephropathy. Methods Rate immunonephelometry assay was used to determine 45 cases of type 2 diabetetes and 18 controls, and the results were compared. Results 24h urine transferrin (TRF) in three different stages of diabetic nephropathy, normal protein urine group, microabumin urine group and clinical protein urine group was 2.73±0.26mg/L,8.52±2.15mg/Land 45.25±16.42mg/L respectively,which was significantly higher than that of the controls(TRF was1.85±0.23mg/L), and TRF was correlated positively with urine microabumin. Conclusion The conscentration of urine TRF can reflect the early damage of the kidney of diadetes, and was more sensitive and appeared earlier than urine microalbumin.
