脑梗塞患者血小板参数的变化及意义

Change and Clinical Significance of Platelet Parameters in Cerebral Infarction

目的 探讨血小板 5项参数对脑梗塞患者的临床意义。方法 分别测定 10 8例脑梗塞住院患者和 4 0例中老年体检者的血小板数 (PLT)、平均血小板体积 (MPV)、血小板体积分布宽度 (PDW)、血小板比积 (PCT)和血小板最大聚集率 (MAR) ,对血小板 5项参数进行比较并作相关分析 ,同时对脑梗塞组中 4 0例患者治疗前后的 5项参数进行配对比较。结果 脑梗塞组PLT减少、MPV和MAR增高较对照组有显著差异 (P <0 .0 1,0 .0 5 )。PDW、PCT两组间无显著差异。对照组PLT与MPV为中度负相关 (r =- 0 .5 5 ,P <0 .0 1) ,脑梗塞组二者为低度负相关 (r =- 0 .37,P <0 .0 5 )。脑梗塞组治疗后MPV减小 ,有显著差异 (P <0 .0 5 ) ;PLT、PDW、PCT和MAR无显著差异。结论 MPV增大是脑梗塞的独立危险指标 ,观察PLT、MPV和MAR的变化对脑梗塞的诊断和预防有指导意义。

Objective To explore the clinical significance of five platelet parameters in patients with cerebral infarction.Methods The levels of platelet count (PLT),mean platelet volume (MPV),platelet volume distribution width(PDW),plateletcrit(PCT) and max aggregation rate(MAR) of 108 cases of the cerebral infarction group and 40 cases of the control group,collected from Dec 2001 to Jan 2003 were measured. Five platelet parameters of the cerebral infaraction group and the control group were compared, and their correlation coefficient tested. The parameters of 40 cases of the cerebral infarction group were compared,before and after treament.Results PLT in the cerebral infarction was significantly less than that in the control group(P<0.05), MPV and MAR were significantly increased (P<0.01) between two groups but no significant difference in PDW and PCT. MPV was decreased after treatment, but there were no significant deference in PCT, PLT, PDW and MAR. PLT and MAR has negative correction in the control group and the cerebral infarction. Conclusion The increasment of MPV is a dangerous marker, observe the change of the PLT, MPV and MAR is important to diagnose and prevent the cerebral infaraction group.
