摘要
目的研究实验性眼后节穿通伤增生性玻璃体视网膜病变(PVR)玻璃体中细胞增殖情况及意义.方法 32只青紫蓝兔组制备外伤性PVR模型后随机分为四组,在伤后第3、7、14、21 d各抽取0.3 ml玻璃体进行细胞计数并用流式细胞仪(FCM)计算出增殖细胞比例.定期观察眼底情况并选择照相.结果在伤后第3、7、14、21 d玻璃体内细胞增殖指数分别为4.82%、13.21%、18.21%、31.48%;增殖细胞密度分别为2.862×103/ml、8.725×103/ml、12.249×103/ml、18.823×103/ml.伤后7 d内无明显的牵引性视网膜脱离(TRD)出现,在第35 d,TRD的发生率为81.3%(26/32).第7 d的增殖细胞数可以预测第35 d的TRD发生情况.结论 FCM分析玻璃体内增殖细胞数量可用来早期筛选高危PVR,反映PVR的预后.
Objective To investigate vitreous cellular proliferation in an experimental model of traumatic proliferative vitreoretinopalhy( PVR). Methods Experimental model of traumatic PVR was induced in pigment rabbits. Then the rabbits were randomly divided into 4 groups. Animals were examined by indirect ophthalmoscopy at weekly intervals for 5 weeks and fundus photographs were selectively taken. Vitreous specimens were obtained at 3,7,14 and 21 post-injury day to analyze the cell number by hemocytometer and flow cytometry. Results Twenty-six of 32 rabbits (81%) developed fractional retinal detachment. At day 3,7,14 and 21 ,the cell proliferation index in the vitreous was 4. 82% ,13. 21% ,18. 21% and 31. 48% ; the proliferating cells was 2.862 × 103/ml, 8.725 × 103/ml, 12.249 × 103/ml, 18.823 × 103/ml respectively. The nature and magnitude of tractional retinal detachment 5 weeks later could be predicted based on the proliferating cell at day 7. Conclusions Analysis of the vitreous proliferating cells is highly predictive of ultimate outcome.
