凡能斯提尔美容切口在妇产科手术中的应用

Application of aesthetic Pfannenstiel incision in obstetrics and gynecology laparotomy

目的:探讨凡能斯提尔(Pfannenstiel)美容切口在妇产科手术中应用的可行性。方法:回顾性分析我院妇产科2001年1月至2002年12月,对良性妇产科疾病实施手术过程中,分别应用Pfannenstiel美容切口腹直肌前鞘弧形切开法385例(组1)、Pfannenstiel切口腹直肌前鞘纵形切开法293例(组2)及下腹纵形正中切口272例(组3)。结果:组2及组3切口并发症如:脂肪液化、感染及裂开发生率增加,与组1比较,切口并发症发生率差异显著(P<0.01)。结论:Pfannenstiel美容切口腹直肌前鞘弧形切开法切口并发症少,并达到了术后美观的效果,适宜在妇产科良性疾病手术中应用。

Objective This study was done to evaluate the feasibility of the application of aesthetic Pfannenstiel incision in obstetrics and gynecology laparotomy for benign diseases. Methods Retrospective data analysis about complications of different incisions included 385 aesthetic Pfannenstiel incision by arch incision at front sheath of rectus (group 1) and 293 aesthetic Pfannenstiel incision by longitudinal incision at front sheath of rectus (group2) and mid-line longitudinal incisions at subabdominal wall(group3) of obstetrics and gynecology laparotomy performed for benign diseases in our hospital from January 2001 to December 2002. Results Incision complications such as infection, fat liquescence and dehiscence were increased in group one and group two. Rates of incision complications were significant difference compared with group one (P<0. 01 respectively). Conclusion Aesthetic Pfannenstiel incision by arch incision at front sheath of rectus had advantages both from incision complications decreased as well as the aesthetic of view on the application of obstetrics and gynecology laparotomy for benign diseases.
