彩超对主动脉夹层动脉瘤的诊断价值

Diagnostic Value of Color Doppler Ultrasound for Aortic Dissecting Aneurism

目的 评价彩超对主动脉夹层动脉瘤 (ADA)的诊断价值。方法 分析经 CT,MR及手术证实的2 3例 ADA声像图特征及血流动力学变化。结果 彩超诊断 ADA敏感性 1 0 0 % ,特异性 96 .5 % ,准确性 95 .8% ,I型 3例 ,II型 1 6例 ,III型 4例 ,其中原发腹主动脉 1例 ,所有病例均能区分真假腔 ,检出内膜破口 1例。结论 彩超对 ADA分型 ,真假腔的区别具有重要价值 ,是无创性检查 ADA的首选方法 。

Objective To evaluate the diagnostic value of color Doppler flow image(CDFI)for aortic dissecting aneurism(ADA).Methods The ADA image characteristics and color flow chnages in 23 cases with ADA were verified by CT.MR and operation and analysed.Results The sensitivity,specificity and diagnostic accuracy of ADA on CDFI were 100%,95 6%,and 95 8% respectively.Three cases belonged to:3 case type Ⅰ;16 cases,type Ⅱ and 4 cases,type Ⅲ.One case was primary ADA in abdominal aorta.The true and false lumina can be distinguished in all cases.The broken orifice of tunica intima in 1 case was found.Conclusions CDFI has significant value for classifying ADA and for distinguishing ture or false lumina,and it is the first choice method without invasion,but it has low detection rates to find the broken orifice of tunica intima on CDFI.
