耐药表型GST-π、P-gp及TopoⅡ在大肠癌中的表达及其临床意义 被引量:9

摘要 目的 探讨大肠癌组织中胎盘型谷胱甘肽 S 转移酶 (GST π)、糖蛋白 (P gp)和DNA拓扑酶Ⅱ (TopoⅡ )的表达与临床病理和预后的关系。方法 采用免疫组化方法检测 170例大肠癌组织中GST π、P gp和TopoⅡ的表达 ,对其组织病理改变和 5年生存率进行综合分析。结果 在高、中分化及低分化大肠癌中 ,GST π阳性率分别为 56 .3%、6 5.6 %和 93.8% ;TopoⅡ阳性率为 2 5.0 %、32 8%和 6 8.8% ,P gp阳性率为 37.5%、50 .0 %和 75.0 %。GST π和TopoⅡ表达在高、低分化两组间及中、低分化两组间均有非常显著的差异 (P <0 .0 1) ,P gp表达均有显著差异 (P <0 0 5)。而在不同的年龄、性别、Duke’s分期及有无淋巴结和远处转移中 ,GST π、P gp、TopoⅡ的表达均无显著差异 (P >0 0 5)。5年内生存者GST π、P gp和TopoⅡ表达阳性率低于 5年内死亡者 ,GST π和P gp有显著差异(P <0 0 5) ,TopoⅡ有非常显著差异 (P <0 0 1)。结论 耐药表型GST π、P gp。 Objective To investigate the relationship between t he expression of glutathione-s-transferase-π(GST-π), P-glycoprotein(P-gp)a nd DNA topoisomerase(Topo Ⅱ) and pathological characteristics and prognosis in colorectal cancer. Methods The expression of GST-π, P-gp, Topo Ⅱ in 170 cases of the colorectal cancer was detected by immunohistochemical staning. Their corr elation with histopathology and 5-year survival rates were generally analyzed. Results The expression rates of GST-π, P-gp, Topo Ⅱ were 56 .3%, 37.5% and 25.0% respectively in the well differentiation tumour tissues, 65 .6%, 50.0% and 32.8% in the middle differentiation tumour tissues and 93.8%, 75. 0% and 68.8% in the low differentiation tumour tissues. There were significant d ifferences in the expression of GST-π, Topo Ⅱ (P<0.01) and P-gp (P<0 .05) between well or moderate differentiation tumour and low differentiation tum our. No significant differences were found between the expression of GST-π, P -gp and Topo Ⅱ and different age, sex, Duke's classification and lymph node or distant metastasis (P>0.05). The expression rates of GST- π, P-gp and Top o Ⅱ in the patients who were alive more than 5 years were lower than that of th e dead. There were obvious and significant differences in the expression of GST -π, P-gp (P<0.05) and Topo Ⅱ (P<0.01). Conclusions Expression of GST-π, P-gp, Topo Ⅱ can be regard ed as a valuable indicators of prognosis of colorectal cancer.
