摘要 目的:探讨血清中肝癌相关性甘氨酰脯氨酸二肽氨基肽酶同工酶(GPDA-F)定量检测对原发性肝癌的诊断价值。方法:利用聚丙烯酰胺凝胶电泳法分离GPDA-F后进行扫描定量。同步检测肝癌和不同良性肝病患者血清GPDA-F、丙氨酸氨基转移酶(ALT)和甲胎蛋白(AFP),分析GPDA-F活性与肿瘤大小、AFP水平及ALT之间的关系,比较GPDA-F定性和定量检测对肝癌的诊断价值。结果:肝癌患者GPDA-F活性明显高于良性肝病患者(P<0.01),GPDA-F活性超过诊断界值者在肿瘤<5cm的肝癌和AFP阴性(<50滋g/L)肝癌分别达60.0%和71.4%,GPDA-F/ALT比值对肝癌的诊断特异性和准确率均高于GPDA-F定量检测。结论:GPDA-F定量检测可提高对肝癌的诊断水平。 Objective: To study the value of quantitative determination of hepatoma-associated glycylproline dipeptidyl aminopeptidase (GPDA-F) in the diagnosis of primary hepatic carcinoma (PHC). Methods: GPDA-F was separated by polyacrylamide electrophoresis and then quantified by scanning. Serum GPDA-F, alanine aminotransferase (ALT) and alpha-fetoprotein (AFP) were simultaneously measured in PHC and other liver diseases. Relationships between GPDA-F activities and tumor sizes, AFP concentrations as well as ALT activities were analyzed. The values of quantitative and qualitative determinations of GPDA-F for the diagnosis of PHC were compared. Results: GPDA-F activities were significantly higher in PHC than benign liver diseases (P<0.01). GPDA-F activities were over cut-off in 60.0% PHC with tumor sizes less than 5cm and 71.4% PHC with negative AFP(<50?滋g/L). The specificity and accuracy of GPDA-F/ALT were higher than those of GPDA-F quantification for the diagnosis of PHC. Conclusion: Quantitative determination of GPDA-F may improve its value for the diagnosis of PHC.
