病灶切除与热灼联合术在难治性癫痫治疗中的应用(英文)

Application of lesionectomy combined with bipolar coagulation on the treatment of intractable epilepsy

目的 研究病灶切除与热灼联合手术方法治疗继发性、难治性癫痫的临床效果。方法 对 2 91例继发性、难治性癫痫患者采取病灶切除与热灼联合手术的治疗方法 ,并随访观察此种联合手术方法对难治性癫痫的控制效果。结果 采用病灶切除与热灼联合手术后 ,2 91例患者中 ,有 2 11例(72 .5 % )发作消失 ,有 5 0例 (17.2 % )发作减少 5 0 %以上 ,对癫痫总的有效控制率为 89.7% ,随访 1.5～ 4 .0年 ,平均 2 3个月 ,2 11例发作消失的患者中有 83例 (39.3% )已停服抗癫痫药 ;同时 ,病灶切除与热灼联合术无永久性的术后并发症。结论 病灶切除与热灼联合术治疗继发性、难治性癫痫 。

Objective To observe the clinical effects of lesionectomy combined with bipolar coagulation on the cortexes with epileptogenic foci in intractable epilepsy.Methods Two hundred and ninety-one cases of patients were managed by lesionectomy combined with bipolar coagulation and the rate of epilepsy-control after operation was studied.Results Among 291 cases of patients, seizures of 211 (72.5%) cases disappeared and paroxysm of 50 ( 17.2%) cases decreased by 50% and more. The general rate of epilepsy-control was 89.7%. The follow-up survey, which had lasted for 1.5 to 4.0 years, averaged 23 months, showed 83 (39.3%) cases in all patients whose seizures disappeared had already stopped taking anti-epilepsy drugs. Meanwhile, lesionectomy combined with bipolar coagulation had no post-operative complication.Conclusion Lesionectomy combined with bipolar coagulation is a safe management to treat intractable epilepsy and its clinical effect is satisfactory.
