运用去唾液酸糖蛋白受体定量评估肝脏储备功能体系的建立 被引量:3

摘要 目的 肝脏去唾液酸糖蛋白受体(ASGPR)指标结合吲哚氰绿储留率(ICGR_(15))建立综合评估肝脏储备功能的体系。方法 测定不同比例肝脏切除大鼠模型肝脏ASGPR的HH_(15)、LHL_(15)和MRI结合ICGR_(15),建立一个综合定量评估肝脏储备功能的评分体系。结果 在正常肝脏容量丢失15%时,HH_(15)、LHL_(15)、MRI、ICGR_(15)等均不能显著反映出肝脏储备功能的变化,而涵盖上述指标的综合评估体系Y值(肝脏功能残余量)则能显著反映肝脏储备功能的变化。结论 涵盖HH_(15)、LHL_(15)、MRI及ICGR_(15)等指标的回归方程Y=2.56+33.188×MRI-44.844×HH_(15)+24.032×LHL_(15)-34.915×ICGR_(15)中的Y值是评估肝脏储备功能的敏感指标,能够对四氯化碳肝硬化大鼠模型的肝脏储备功能作出准确可靠的定量评估。 Objective To establish a method for quantitative assessment of hepatic functional reserve. Methods An integrative scale system of evaluating hepatic functional reserve quantitatively was established by analysis of rat liver asialoglycoprotein receptor indices (HH15 , LHL15 , MRI) and ICGR15 after hepatotectomy of different proportion. Results Such indices as HH15 , LHL15, MRI, ICGR15 could not reflect the change of hepatic functional reserve when the losing of liver capacity reached 15% , but the value of integrative coeiEcient Y (remain of liver functional) including those indices can reflect the change. Conclusions The integrative coefficient Y in regression equation Y = 2.56 + 33.188 x MRI - 44.844 x HH15 + 24.032 x LHL15 - 34.915 x ICGR15 including HH15 , LHL15, MRI and ICGR15 is the the most sensitive index of evaluating hepatic functional reserve quantitatively . It can evaluate the hepatic functional reserve quantitatively and accurately in the models of rat liver cirrhosis induced by carbon tetrachloride.
