皮肌炎与硬皮病皮损原位细胞凋亡研究

摘要 目的 : 探讨角质形成细胞凋亡在皮肌炎 (DM)和系统性硬皮病 (SSc)皮损发生发展中的作用。方法 : 采用原位细胞凋亡检测 (TUNEL)方法研究两病患者皮损角质形成细胞的凋亡情况。结果 :  (1)性别、年龄、病程、疗前评分、疗后评分、住院天数、糖皮质激素总量等因素 ,对DM和SSc的凋亡指数的影响均无统计学意义 (P均 >0 .0 5 ) ;(2 )正常对照 8例表皮细胞凋亡指数均 <0 .0 0 5 ,DM与SSc凋亡指数均明显高于对照组 (P均 <0 .0 1) ;(3)角质形成细胞凋亡发生在整个皮损区的角质形成细胞。结论 : 角质形成细胞凋亡异常与DM和SSc皮损的发生可能有密切联系。 Objective: To investigate the role of keratinocytes apoptosis in the occurrence and development of skin lesions in dermatomyositis (DM) and systemic sclerosis (SSc). Methods: The keratinocytes apoptosis in DM and SSc and control subjects were measured by terminal deoxynucleotidyl transferase-mediated dUTP nick end labeling (TUNEL).Results:(1) Sex, age, course of diseases, scores of diseases before and after treatment, the duration in hospitalization and total amount of corticosteroid were not significantly associated with the index of apoptosis in DM and SSc (P>0.05). (2) The index of apoptosis in control subjects was less than 0.005, while the index was higher in DM and SSc than in control subjects(P<0.01). (3) In DM and SSc, TUNEL-stained apoptotic keratinocytes were present in the whole region of the skin lesions. Conclusion: The abnormality of apoptotic keratinocytes may be closely related to the occurrence of skin lesions in DM and SSc.
