不稳定型心绞痛患者循环中NF-κB检测及价值探讨 被引量:4

摘要 目的 探讨核因子κB(NF κB)水平与不稳定型心绞痛 (UAP)危险度分层的关系。方法 采用酶联免疫吸附法 (ELISA)定量测定外周血白细胞中NF κB水平。 5 7例入选病人分稳定心绞痛 (SAP)和UAP两组 ,UAP组按Braunwald分级进行危险度分层。结果 SAP组、Braunwald 1级、2级、3级组NF κB分别为 0 5 89± 0 390、0 933± 0 194、1 10 3± 0 14 7、1 348± 0 181。UAP各组NF κB与SAP组比较均有显著差异 (P <0 0 1)。NF κB与UAPBraunwald级别及心绞痛发作时心电图ST段电压变化、冠状动脉复杂病变、住院期间心绞痛再发作呈正相关 (P <0 0 5~ 0 0 0 1)。与射血分数 (EF)、冠脉粥样斑块病变数、胆固醇 (Tch)、三酰甘油 (TG)、高密度脂蛋白胆固醇(HDL C)、低密度脂蛋白胆固醇 (LDL C)、高血压、糖尿病、吸烟指数、体重指数 (BMI)无明显相关关系(P >0 0 5 )。结论 NF κB与UAP危险度分层呈正相关 。 Objective To investigate the relationship between NF-κB and risk classification of unstable angina pectoris.Methods 57 patients were divided into two groups:stable angina pectoris group(SAP)and unstable angina pectoris group(UAP).Braunwald classification was used to evaluate the risk for cardiovascular events in patients with unstable angina pectoris.NF-κB was measured with ELISA.Results The levels of NF-κB were 0 589±0 390 in SAP group,0 933±0 194 in Braunwald 1 group,1 103±0 147 in Braunwald 2 group and 1 348±0 181 in Braunwald 3 group.The NF-κB activity in each group of UAP was significantly elevated compared with SAP group(P<0 01).The level of NF-κB was positively correlated with the risk classification of unstable angina pectoris,the depression or elevation of ST segment of ECG,unstable plaques and chest pain epsodes in admission period but had no correlation with EF,Tch,TG,HDL-C,LDL-C,hypertension history,diabetes mellitus history,smoking index,BMI and sum of atherosclerosis plaques(P>0 05).Conclusion The activity of NF-κB was positively correlated with the risk classification of unstable angina pectoris and can be regarded as a useful indicator reflecting the risk for cardiovascular events in patients with unstable angina pectoris.
