住院患者病情危重度分级研究概述

An Account on the Research of the Classification and Evaluation of the Severity of In-Patients'(5 Pieces)

目的 以统计分析法为主 ,建立覆盖所有病种的住院患者病情分级方案 ,提出基于患者个体和群体的危重度表达指标。方法 用大样本住院患者的病案首页数据 ,采用决策树分析法 ,探索各种疾病患者的病情分类规则。结果 建立了 16 3个基于病种的病情分类模型 ,产生了 6 36个危重度类别和危重度分值 ;以医院为单位计算了患者病情危重度指数。经新样本考核 ,这些指标对治疗结果、住院费用有一定预测能力 ,且与医院规模相关。结论 病情危重度分级方案能够反映住院患者病情的相对危重度 ,可作为病情测量的有效工具 。

Objective To classify and evaluate the severity of inpatients by statistic method, and provide indicators presenting severity. Methods By application of decision tree with large abstract data of inpatients, discovery the classification pattern of patients based on their severity is to be studied. Results 163 models were established based on the categories of diseases with 636 severity groups and numeric severity score. Severity Index, which indicates the severity of patients as a group was also calculated. The validation result by applying the model to new data showed that these indicators could predict clinical outcome and distinguish hospitals of different scales to a certain extent. Conclusions This classification model can indicate the severity of inpatients and thus can be used as a tool in management of medical cost and quality of healthcare.
