摘要
目的 :研究探讨多层螺旋CT血管造影 (简称 :MSCTA)在脑血管性疾病诊断中的临床应用价值。方法 :收选在我院行MSCTA检查的脑血管性疾病 2 2例 ,使用GELightSpeedPlus四层螺旋CT机 ,螺距 :3~ 6 ,层厚 :1.2 5mm ,注药流率 :2 .5~ 4 .0ml/s ,成人剂量 :10 0ml,儿童按 2ml/公斤计算 ,扫描延迟时间 :15~ 2 2s。图像后处理使用ADW 3.1工作站进行脑血管二维及三维重建。结果 :MSCTA可以立体、直观地显示脑血管的空间解剖细节 ,可充分观察到脑动脉瘤的准确起源、瘤体大小、颈部宽窄及走向等情况 ,其中 8例手术病例结果与MSCTA诊断结果一致 ,本组病例中所显示的最小动脉瘤直径约 3mm ;同时 ,MSCTA也可作为脑动脉瘤术后评价的一种无创性检查方法 ,观察术后银夹所在的位置和远端血管通畅情况等。三维图像对于脑动静脉畸形、海绵状血管瘤和烟雾病等也具有良好的诊断价值。三维成像中 ,容积显示图像为最佳。结论 :MSCTA是一种无创性成像方法 ,在脑血管性疾病的诊断和术前术后评价中发挥着重要作用。
Objective:To study and discuss the clinical value of multi-slice helical CT angiography (MSCTA) in detection of cerebral vascular diseases.Methods:Selecting twenty-two new cases in our hospital concerning cerebral vascular diseases imaging with GE Light Speed Plus four slice helical CT,pitch:3~6,slice thick:1.25mm,injecting adults dosage:100ml,injecting children dosage:2ml/kg,injecting rate:2.5~4.0ml/s,scaning delay time:15~22s.Scanning images were transferred into ADW 3.1 work-station to create three-dimensional cerebral vascular images.Results:MSCTA can clearly display spacial anatomize details so that we can accurately observe the source,size and neck's width,route of cerebral aneurysm.MSCTA results of eight cases are in accordance with those operations.The smallest diameter shown in our experiment is about 3mm.At the same time,MSCTA could perform post-operational evaluations of cerebral aneurysm as a harmless examination method,with observing the locatin of silver clip and the far vessel.Besides,MSCTA could be used to diagnose arteriovenous malformation,sponge aneurysm,aerosol sick.Volume rederening (VR) is the best in the three-dimensional images.Conclusion:As a harmless examination method,MSCTA plays an important role in detection,pre-operational and post-operational evaluation of brain vascular disease.
