摘要
目的 :探讨新生儿缺氧缺血性脑病 (HIE)血清IL 8、IL 1 0及神经元特异性烯醇化酶 (NSE)变化及临床意义 ,为新生儿HIE的诊断、治疗及判断预后提供理论依据。方法 :选择 35例HIE新生儿为研究对象(HIE组 ) ,根据病情分为轻度、中度和重度三组 ;同时选择 2 0例同期出生的正常健康足月儿为正常对照组。HIE组分别于急性期及恢复期、对照组于生后 1d采集外周静脉血 ,采用放射免疫测定法 (RIA)和电化学发光免疫分析法 (ECLI)分别检测血清IL 8、IL 1 0和NSE的水平变化。结果 :HIE组急性期血清IL 8、NSE水平显著高于正常对照组 (P <0 0 5 ) ,IL 1 0水平显著低于正常对照组 (P <0 0 5 ) ;重度HIE组血清IL 8、NSE水平明显高于中度和轻度组 ,而IL 1 0水平则明显低于中度和轻度组 ;恢复期血清IL 8、NSE水平明显高于中度和轻度组 ,而IL 1 0水平则明显低于中度和轻度组 ;恢复期血清IL 8、NSE水平仍高于正常对照组 (P <0 0 5 ) ,IL 1 0水平仍低于正常对照组 (P <0 0 5 ) ;HIE血清NSE浓度与IL 8呈正相关 ,与IL 1 0呈负相关。结论 :IL 8、IL 1 0、NSE可能参与了HIE的发生发展过程 。
Objective:To evaluate the dynamic changes of IL 8,IL 10 and neuron specific enolase(NSE) in neonates with hypoxic ischemic encephalopathy(HIE) and their clinical significance to provide basis for the diagnosis,therapy and prognosis of HIE.Methods:Thirty five cases of infants with HIE were selected and divided into three groups according to the severity of the patients' condition,with 20 cases of healthy newborn babies as control group.Peripheral venous blood in the acute and restorative stags of the HIE group was collected and that of the control group at the first day after birth.The dynamic changes of serum IL 8,IL 10 and NSE were measured by RIA and ECLI .Results:The HIE group's levels of IL 8 and NSE are higher in the acute stage than those in the control group( P <0 05),while the lever of IL 10 is obviously lower than that in the control group( P <0 05).There are significant differences between the levels of IL 8,IL 10 and NSE with different severity in acute stage of HIE.The levels of IL 8 and NSE in the severe HIE group are obvioulsy higher than those in the light and moderate HIE goups ( P <0 05),while the level of IL 10 in the severe HIE group is lower than that in the light and moderate HIE groups ( P <0 05).The levels of IL 8 and NSE are still higher in the restorative stage than those in the control group( P <0 05),while the level of IL 10 is still lower than that in the control group ( P <0 05).The concentration of serum NSE has a positive correlation with IL 8,but has a negative correlation with IL 10.Conclusion:IL 8,IL 10 and NSE may be involved in the occurrence and deveplopment of HIE.The levels of IL 8,IL 10 and NSE are closely correlated with the severity of HIE.
