晚孕妇双重免疫阻断HBV宫内传播的临床观察

A Clinical Observation on Double Immunal Interdiction of HBV's Transmission in Uterus of Pregnant Womem in Their Late Period

目的 :探讨阻断乙型肝炎病毒 (hepatisBVirus,HBV)宫内传播的有效途径。方法 :将乙型肝炎孕妇 1 0 0例为A组于孕 2 8W、32W、36W、4 0W分别给予肌肉注射乙型肝炎免疫球蛋白 (hepatitisBimmuneglobulin ,HBIG) 1 0 0IU及皮下注射重组酵母乙型肝炎疫苗 1 0 μg ,另乙型肝炎孕妇 1 0 0例为B组未行任何干预措施。两组新生儿出生时即取脐血查乙型肝炎病毒标志物 ,观察新生儿窒息率及出生体重。结果 :A组新生儿HBsAg携带率显著低于B组 (P<0 .0 1 ) ;其抗 -HBs阳性率显著高于B组 (P <0 .0 1 ) ;A组新生儿大三阳率显著低于B组 (P <0 .0 1 ) ;两组新生儿窒息率及出生体重无统计学意义。结论 :晚孕妇联合注射HBIG及乙肝疫苗能有效降低HBV的宫内传播 ,对新生儿无影响。

Objective:To discuss the effective ways which interdict the HBV's transmission in uterus.Methods:A group included 100 pregnant women with Hepatitis B.Each of them was injected Hetapitis B immune globulin(HBIG)100IU in muscular tissue and Reorganizational yeastHepatitis B vaccine 10μg in subcutaneous tissue respectively on the 28W,32W,36W,and 40W of their pregnant period.B group included 100 prenant women also with Hepatitis B,but none of intervention measures was taken to them.Each of the neonates in two groups was taken omphalo-blood out of the body,in order to check Hepatitis B Virus marking material.At the same time,the neonate's suffocated rates and born weights were observed,Results:The bearing rates of HBsAg in Group A was lower than in Gorup B significantly( P <0.01).Their Anti-HBs positiverates was higher than that in Group B significantly( P <0.01).The rates of the neonates who are positive of HBsAg,HBeAg and Anti-HBc in Gorup A was lower than that in Group B( P <0.01).The difference of the suffocated rate and born weight between two groups had no statistics meaning.Conclusion:HBIG and Hepatitis B Vaccine can be injected combinedly to the pregnant women with Hepatitis B in their later period It has an effect on decreasing HBV's transmission in the uterus,but has no badeffect on their neonates.
