多普勒超声检测上腔静脉血流速度评价慢性阻塞性肺病患者肺动脉压的临床研究 被引量:2

摘要 目的 探讨多普勒超声检测上腔静脉 (SVC)血流速度评价慢性阻塞性肺病 (COPD)患者肺动脉压的可行性。方法 COPD患者 3 6例 ,均有中度或中度以上的三尖瓣返流。连续多普勒超声测量右心室 右心房压差 (△PRV RA) ,肺动脉高压定义为△PRV RA>2 5mmHg ;于胸骨上窝右肺动脉长轴切面采用脉冲多普勒超声获取上腔静脉血流频谱 ,分别测量吸气相上腔静脉最大前向血流速度 (VINS)和呼气相上腔静脉最大前向血流速度 (VEXP) ,并计算VINS、VEXP比值 (VINS/VEXP)。结果 与无肺动脉高压组比较 ,肺动脉高压组△PRV RA显著增高 ,吸气相上腔静脉最大前向血流速度与其无显著性差异 ,但呼气相上腔静脉最大前向血流速度和VINS/VEXP则显著增高 (P <0 .0 5 )。结论 上腔静脉前向血流速度随呼吸变化是评价COPD患者肺动脉压的有效指标。
