原发性肝细胞癌肿瘤血管生成与增强MRI表现的相关性初探

摘要 目的 研究原发性肝细胞癌的肿瘤血管生成 (微血管密度 ,MVD ;血管内皮生长因子 ,VEGF)与增强MRI的表现的关系。方法 自 1998年 6月至 2 0 0 2年 9月手术切除经病理证实的PHCC 2 6例 ,常规石蜡包埋 ,对石蜡组织块进行 5 μm连续切片 ,采用免疫组织化学EnVisionTM法染色并测定微血管密度及血管内皮生长因子染色 ,并与术前患者的MRI对比 ,研究其相关性。结果 在增强图像中 ,明显强化组MVD值 ,高于中等强化和轻度强化或无强化组 ,但中等强化组和轻度或无强化组之间无差异。VEGF的表达与HCC强化程度有一定关系 ,明显强化的患者组中VEGF的阳性率明显高于中等强化和轻度或无强化组 ,有统计学差异 (P <0 .0 5 )。结论 增强MRI的表现与PHCC的血管生成具有相关性。VEGF的表达可以解释增强MRI的表现 ,MRI可以用于判断血管活性和新生血管形成的情况。 Objective To study the correlation of the enhanced patterns on MRI and PHCC angiogenesis (microvessel density, MVD and vascular endothelial growth factor VEGF).Methods MRI examinations of SET 1W,FSET 2W and enhanced SET 1W sequence were performed in all cases with 0.5T ELSCNT Gyrex MR. All sequences were performed with cross section. Specimens were stained immuno-histochemically.MVD was highlighted by anti-CD34. The enhancement imaging features of PHCC lesions were correlatively studied with tumor MVD,VEGF. Results MVD was higher in the group of significant enhancement (219.56±70.96)than those in the groups of medium enhancement(158.16±65.11) and slight enhancement(151.15±98.67)( P <0.05). VEGF expression was higher in the group of significant enhancement(72.72%)than those in the groups of medium enhancement (33.33%) and slight enhancement (33.33%)( P <0.05). No significant correlation was found between VEGF expression and enhancement features of portal vein phase.Conclusion The enhanced patterns on MRI are relative to PHCC angiogenesis,the expression of VEGF may explain the findings of MRI of PHCC, and MRI is a valuable means to access the angiogenis activity and tumor neovasculaity in vivo for PHCC patients.
