P波离散度与冠心病并发阵发性房颤的关系 被引量:2

摘要 目的 探讨P波离散度(Pd)对中年冠心痛患者并发阵发性心房颤动(PAf)的预测价值。方法 观察42例(年龄在45~65岁)冠心病并发PAf患者的Pd,最大P波值(Pmax)、左房内径(LAD),并与无PAf的同年龄组冠心病患者的Pd、Pmax、LAD比较。结果PAl组Pmax、Pd增高,LAD与对照组有明显差异。Pd≥40ms预测PAf的敏感性为83.3%,特异性为85.7%,阳性预测率为87.8%,阴性预测率为83.7%;Pmax≥110ms预测PAf的敏感性83.3%,特异性为83.9%.阳性预测率为81.4%。阴性预测率83.7%。LAD在PAf组与对照组间无明显差异。结论 体表心电图P波离散度是预测中年冠心病患者并发PAl的一个可靠指标。
