摘要 目的 探讨颅脑损伤后脑血管自动调节功能 (CA)的状态及其临床意义。方法  2 2例颅脑损伤患者在伤后 2 4h内和伤后 6d行经脑多谱勒 (TCD)结合压颈试验来检测CA。结果 伤后 2 4h内CA受损在重型颅脑损伤中有 13例 ,占 81.3% ,和预后呈正相关 ,中型中有 2例 ,占 33.3%。CA在伤后处于不断变化中 ,6d后明显好转。结论 通过对CA的检测 ,可了解颅脑损伤后脑组织的代偿能力 ,并指导临床治疗和判断预后。 Objective To evaluate the cerebral autoregulation(CA) in head-injury patients.Methods CA were studied within 24 hours and on the 6 th day after head injury in 22 patients,by using transcranial doppler recording combining with carotid artery compression test.Results Impaired CA was demonstrated in 13(81.3%) of 16 patients with severe head injury,correlated with outcome,while 2(33.3%) of 6 cases with moderate head injury.The state of CA varied with time,usually restored after appropriated treatment.Conclusions Testing the state of CA let us know the compensation capacity of cerebral tissue after head injury,help us to estblish the CA-oriented therapy,and to predict the outcome.
