电视腹腔镜诊治卵巢妊娠13例分析

Analysis of laparoscopic diagnosis and treatment for 13 cases with ovarian pregnancy

目的 探讨电视腹腔镜下诊治卵巢妊娠的适宜方法。方法 对 13例经电视腹腔镜诊治的原发性卵巢妊娠进行分析。结果 13例卵巢妊娠中 6例患者带有宫内节育环 ,1例继发于IVF -ET治疗 ,7例经镜下手术治疗 :1例卵巢部分切除 ,6例妊娠包块剔除术。镜下手术的患者妊娠囊直径 1.5～ 5 .0cm ,盆腔内出血量 4 6 .87ml,手术时间平均 5 0min ,术后住院天数 3.6d。镜下诊断后开腹手术 6例。结论 腹腔镜适用于早期卵巢妊娠的诊断与治疗 ,相对缩短住院时间 ,保留了卵巢功能 ,是治疗卵巢妊娠的一种有效方法。

Objective:To study the effective method in diagnosis and treatment of ovarian pregnancy under laparoscopy .Methods:To analyze 13 primary ovarian pregnancy diagnosed by laparoscopy. Results: Of 13 cases of ovarian pregnancy,6 cases were with intrauterine devices. One case was secondary from IVF-ET. Seven cases were treated under laparoscopy: 1 case had partial ovarian section and 6 cases had been removal of ectopic pregnancy cysts. Pregnancy cysts diameter were between 1.5～5 cm.Hemoperitoneum was 46.87ml.Average operation time was 50 min. Post-operative days in hospital were 3.6 days. The next 6 patients were done by laparotomy after laparoscopy diagnosis. Conclusions: Laparoscopy is effective in diagnosis and treatment of the earlier ovarian pregnancy. Hospitalization days can be shorten and ovarian function can be conserved. We consider it an effective method to treat ovarian pregnancy.
