牙周治疗对妊娠期GCF-PGE_2及临床指数的影响

The Effects of Periodontal Therapy on Gingival Crevicular Fluid Prostaglandin E_2 and Clinical Indices in Pregnancy

【目的】探讨牙周治疗对妊娠期牙周临床指数及龈沟液前列腺素E_2的影响。【方法】以17名全身健康的妊娠妇女为研究对象,分别于妊娠早期、中期和晚期测量牙龈指数、菌斑指数和探诊深度,用放射免疫法测龈沟液前列腺素E_2水平。同时对患者行口腔卫生教育,龈上洁治及根面平整。【结果】牙周治疗明显改善了各项牙周临床指数,龈沟液前列腺素E_2水平在妊娠中期和晚期明显降低(P<0.01)。龈沟液前列腺素E_2浓度与各临床指数间的相关性并不显著(P>0.05)。【结论】龈沟液前列腺素E_2水平可作为妊娠期牙周治疗效果的指征。牙周治疗如能贯穿整个妊娠期间,则能有效控制妊娠期龈炎。

[Objective] To investigate the effects of periodontal therapy on clinical indices and gingival crevicular prostaglandin E2(GCF-PGE2) in pregnant women. [Methods] 17 systemical-ly healthy pregnant women were recruited for the study. Their clinical indices including plaque index, gingival index, probing depth were tested in their first, second and third trimesters. The contents of GCF-PGE2 were determined using radioimmunological methods. Periodontal treatment consisting of oral hygine instruction, scaling, root planing were performed at each phase. [Results] Periodontal treatment can improve clinical indices(P < 0.05). There is significant decrease in levels of GCF-PGE2 at the second and third trimester after periodontal therapy (P < 0. 01). Non-significant correlation was found between GCF-PGEiand clinical indices( P > 0.05). [Conclusion] The concentration of GCF-PGEi may be used as a marker of gingival inflamation in order to determine the effects of periodontal treatment in pregnant women. Periodontal therapy that is performed throughout the entire pregnancy period may help prevent the threat of pregnancy gingivitis.
