摘要 目的:总结电子阴道镜辅助高频电波刀诊治宫颈病变的近期效果。方法:电子阴道镜下定位活检宫颈病变229例,同时免疫组化检测人乳头瘤病毒(HPV)。高频电波刀治疗101例,治疗方法分别采用LEEP锥切术(8例)、LEEP活检术(54例)以及超高频射频术(39例),对其疗效进行回顾性分析。结果:镜下定位活检HPV检出率41.5%。高频电波刀施术时间(5.2±0.6)min,愈合率为95%,有效率为99%,无效率为1%。宫颈上皮内瘤变(CIN)患者术后3个月复查,94.4%HPV感染转为阴性,无病变持续存在。结论:高频电波刀治疗宫颈病变手术时间短、术后并发症少、效果良好,是治疗宫颈病变的理想方法,HPV检测有助于宫颈病变的治疗。 Objective: To summarize the clinical experience of using digital colposcopy and high frequency radiosurgical knife (HFRK) for treatment of cervical lesion. Methods: 229 cases with cervical lesion in our hospital were diagnosed after colpscop-ic examination and multiple biopsy. Meanwhile, human papillomavirus(HPV) was studied immunochistochemically in the tissue sections. 101 patients of all cases,who were treated by HFRK under colposcopic guidance for loop electrosurgical excision pro-cedure(LEEP) conization(8 cases) ,LEEP biopsy(54 cases) and radio - frequency(39 cases), were analyzed retrospectively. The optical depth of cervical conization and biopsy was 7mm and 4mm respectively in the cervix, and 15mm and 4mm respectively in the cervical tube. Results: The positive detection rate of HPV was 41. 5%. The average operation duration of HFRK was (5. 2 ± 0. 6)min. 96 cases(95%) were cured,100 cases(99%) effective,1 cased(1 %) of no effect, patients with CIN(18 cases) were followed up in 3 months ,17 cases(94. 4%) with HPV infection turned negative.no patients had persistent CIN. Conclusion: HFRK is an effective and safe treatment aption for cervical lesion. HPV detection will help us treat cervical lesion.
