应用多普勒超声音频处理系统自动估测肺血管阻力的研究 被引量:4

摘要 目的 评价多普勒超声音频处理系统 (DASPS)自动定量估测肺血管阻力 (PVR )的准确性。方法 对 3 2例先天性心脏病患儿 ,应用DASPS和多普勒超声技术 ,根据主动脉、肺动脉、二尖瓣及肺静脉血流时间间期参数估测肺动脉平均压 (PAMP)、肺血流量 (QP)及肺小动脉嵌压 (PAWP) ,按Poiseuille公式PVR =(PAMP PAWP) /QP 计算肺血管阻力 ,并与心导管实测值对比。结果 心导管测量的肺血管阻力与DASPS估测值高度相关 ( r =0 .93 ,P <0 .0 0 1) ,与多普勒超声技术估测值中等程度相关 ( r =0 .79,P <0 .0 1)。DASPS估测肺血管阻力所需时间短于多普勒超声技术所需时间 (P <0 .0 1) ,且重复性较好。 Objective To assess the accuracy of Doppler audio-signal processing system(DASPS) in noninvasive automatic estimation of pulmonary vascular resistance (PVR). Methods Comparative studies among Doppler echocardiography, audio-signal processing system and cardiac catheterization for predicting PVR had been done in 32 children with left-to-right shunting congenital heart disease. Results PVR measured by cardiac catheterization highly correlated with that estimated by DASPS adopting flow time intervals of aorta, pulmonary, pulmonary vein and mitral valve (r= 0.93, P< 0.001), better than that measured by Doppler echocardiography (r= 0.79, P< 0.01). Conclusions PVR could be accurately auto-estimated noninvasively by Doppler echocardiographic audio-signal processing system.
