卵巢过度刺激综合征预测因素的初步探讨 被引量:3

摘要 目的 探讨体外受精 胚胎移植 (IVF ET )中预测卵巢过度刺激综合征 (OHSS )的相关因素。 方法  4 3例IVF ET患者分为 3组 :组 1为中、重度OHSS患者 9例 ,组 2为具有OHSS风险而未发病的 14例 ,组 3为同期各项指标正常的 2 0例患者。从临床资料、血清性激素水平、血及卵泡液血管内皮生长因子 (VEGF)水平综合评价。 结果 组 1与组 2基础卵泡数、人绒毛膜促性腺激素 (hCG)注射日卵泡数、获卵数显著升高 (P <0 .0 5 )。组 1黄体生成素 /卵泡刺激素比值显著升高 (P <0 .0 5 ) ,且hCG日E2 水平显著升高 (P <0 .0 5 )。hCG日组 1、组 2血清VEGF水平显著高于组 3(P <0 .0 5 ) ,而组 1与组 2无显著差异。取卵日组 1、组 2血清VEGF水平显著高于组 3(P <0 .0 5 ) ,且组 1与组 2之间也存在显著性差异 (P <0 .0 5 )。 结论 对临床资料、血清性激素及VEGF水平的变化进行综合分析可提高预测OHSS的准确性。 Objective: To explore the related factors in predicting the risk of ovarian hyperstimulation syndrome (OHSS). Methods: Forty three women undergoing in vitro fertilization were divided into three groups. Group1, 9 patients with moderate or severe OHSS; Group2, 14 patients at risk of OHSS; Group3, 20 patients as normal control. We comprehensively evaluated the action of the clinical data, sex hormone levels, and vascular endothelial growth factor (VEGF) levels in serum and in follicular fluid. Results: The number of follicules and the number of oocyte retrieval were significantly higher in group1 and group2( P <0.05).LH/ FSH rate and E 2 on the day of hCG were significantly higher in group1 when compared with group2 and group3. The concentration of VEGF on the day of oocyte retrieval was significantly higher in group1 than that in group2 and group3 Conclusion: Clinical data, LH/ FSH rate, the serum VEGF and E 2 concentrations may serve as predictable factors for OHSS risk accurately.
