垂体后叶素加硝酸甘油联合治疗咯血疗效分析

Analysis of pituitrin with nitroglycerin in combinative therapy on patients with hemoptysis

目的 评价垂体后叶素加硝酸甘油联合治疗咯血的疗效。方法 应用垂体后叶素加硝酸甘油联合治疗咯血 2 2 3例定为联合治疗组 ,并把同期应用垂体后叶素单药治疗咯血 2 12例定为对照组 ,对比分析其疗效及不良反应。结果 在治疗 4 8小时后显示 :联合治疗组疗效明显比对照组优越 ,差异有统计学意义 ,在控制大咯血时 ,联合治疗组有效率 79.6 % ,对照组有效率 5 4 .9% ,差异有统计学意义 (P <0 .0 1) ;在控制中咯血时 ,联合治疗组有效率72 .9% ,对照组有效率 5 3.1% ,差异有统计学意义 (P <0 .0 1) ;在控制小咯血时 ,联合治疗组有效率 4 1.2 % ,对照组有效率 30 .8% ,差异无统计学意义 (P >0 .0 5 ) ;联合治疗组出现心悸、胸闷、高血压等不良反应明显比对照组少 ,差异有统计学意义 (P <0 .0 1) ,而出现腹痛、腹泻、出汗、面色苍白等不良反应与对照组相差不多 ,差异无统计学意义 (P >0 .0 5 )。结论 垂体后叶素加硝酸甘油联合治疗咯血比垂体后叶素单药治疗咯血疗效明显提高 ,不良反应明显减少。

Objective To assess the curative effect of pituitrin with nitroglycerin in combinative therapy on the patients with hemoptysis.Methods Two hundred and twenty-three patients(combinative group) with hemoptysis received the combinative therapy of pituitrin and nitroglycerin, while 212 patients(control group) with hemoptysis received the single therapy of pituitrin, comparisons were made between two groups in curative effect and adverse reactions. Results After 48 hours' treatment, the total curative effect of combinative group was significantly higher than that of control group. In controlling massive hemoptysis, the effective rates were 79.6% in combinative group and 54.9% in control group(P< 0.01), in controlling moderate hemoptysis, the effective rates were 72.9% in combinative group and 53.1% in control group(P< 0.01), in controlling mild hemoptysis, the effective rates were 41.2% in combinative group and 30.8% in control group(P> 0.05). The attendance of palpitation, stuffiness and hypertension in combinative group was significantly less than that in control group(P< 0.01), but the attendance of abdomen-ache, diarrhea, diaphoresis and pale-looking in combinative group was not significantly less than that in control group (P> 0.05). Conclusion Pituitrin with nitroglycerin is more effective with less adverse reactions than single pituitrin in treating hemoptysis.
