摘要
目的 :动态观察大鼠不同形式脾损伤愈合的组织学变化 ,评估脾破裂保守及保脾治疗的理论依据。方法 :三种脾破裂模型分为 1w、2w、3w、4w、5w动态组织学观察脾脏愈合的过程。结果 :1w为血肿期 ;2w、3w为脾脏结构再生起始期 ,红细胞淤积消散 ,创面充血减少 ,T、B淋巴细胞开始聚集 ;4w、5w为再生期 ,T、B淋巴细胞归位 ,小动脉周围淋巴鞘 (periarteriolarlymphocyticshealths,PLAS)结构逐渐清晰 ,淋巴细胞密度增大 ,出现胶原纤维瘢痕。结论 :脾脏外伤有较好的自愈能力 。
Objective:To observe dynamically histological changes of healing of injury of spleen in different pattern of rats and evaluate the theoretical basis of conservative treatment and conservative operation of splenic rupture.Methods:Three kinds of splenic rupture models were divided into 1 w,2 w,3 w,4 w and 5 w to observe dynamically histology of healing process of the spleen.Results:1 w was hematoma period,2 w and 3 w were initial regenerative periods. Stasis of erythrocytes began to eliminate;hyperemia on the surface of wound decreased,T and B lymphocytes began to gather each other. 4 w and 5 w were regenerative period,at this time,T and B lymphocytes returned to its original position,the structures of periarteriolar lymphocytic sheaths(PLAS) were gradually clear and the density of the lymphocyte was increased and the cicatrix of the collagenous fibril appeared.Conclusion:Injury of spleen has stronger ability of spontaneous cure,the regeneration and the cicatrix of the collagenous fibril are the important cause for spontaneous cure.
