摘要
目的 :研究血尿酸水平与冠心病的关系。方法 :2 12例住院病人 ,分为非冠心病组 (91例 )和冠心病组 (12 1例 ) ;冠心病组又分为单支病变亚组 (5 0例 )、两支病变亚组 (30例 )和三支病变亚组 (41例 ) ;比较在不同分组条件下各组间血尿酸水平的差别。多元逐步回归分析血尿酸水平与冠状动脉病变积分间的相关性 ,Logistic回归分析血尿酸水平与冠心病发病间的相关性。结果 :冠心病组血尿酸水平 [(378± 10 0 ) μmol/L]非常显著高于非冠心病组 [(345± 77) μmol/L](P <0 .0 1) :单支病变亚组、两支病变亚组和三支病变亚组血尿酸水平分别为 (348± 88) μmol/L、 (397± 10 4 ) μmol/L和 (40 7± 10 9) μmol/L ,两支病变亚组和三支病变亚组均显著高于单支病变亚组 (P <0 .0 5 ;P <0 .0 1) ;血尿酸水平与冠状动脉病变积分非常显著相关 (P<0 .0 1) ;Logistic回归分析结果表明 ,调整年龄、性别、BMI、SBP、DBP、PP、TC、TG、HDL、LDL、ApoA1、ApoB、Lp(a)、Glu后 ,血尿酸水平与冠心病的发病间无显著相关性 (P >0 .0 5 )。结论 :血尿酸水平随冠心病人病情严重程度而增加 ,但不是冠心病的独立危险因素。
Objective:To study the relationship between serum uric acid level and coronary heart disease.Methods:212 patients in hospital were divided into two groups of non-coronary heart disease (91 cases) and coronary heart disease (121 cases).The group of coronary heart disease was divided into three subgroups of single-branch,double-branch and three-branch according to the scope of pathological changes.The serum uric acid levels in different groups were compared.The correlation between serum uric acid level and coronary artery lesion was analysed using univariate stepwise regression model.The logistic regression analysis was used to investigate the association of serum uric acid level with the incidence of coronary heart disease.Results:The serum uric acid level in coronary heart disease group[(378±100)μmol/L]was significantly higher than that in non-coronary heart disease group[(345±77)μmol/L](P<0.01).The serum uric acid levels of single-branch subgroup,double-branch subgroup and three-branch subgroup were (348±88)μmol/L,(397+104)μmol/L and (407+109)μmol/L respectively.The serum uric acid levels in double-branch subgroup and three-branch subgroup were obviously higher than that in single-branch subgroup(P<0.05, P<0.01).There was pronounced positive correlation between serum uric acid level and coronary artery lesion(P<0.01).The logistic regression analysis indicated the serum uric acid level was not related to the incidence of coronary heart disease after adjusting age,sex,BMI,SBP,DBP,PP,TC,TG,HDL,LDL,ApoA 1,ApoB, Lp(a) and Glu(P>0.05).Conclusion:The serum uric acid level increases along with the severity of coronary heart disease, but it is not an independent risk factor for coronary heart disease.
