玻璃体切割手术对角膜内皮细胞的影响

Effect of vitrectomy on corneal endothelial cells

目的 观察玻璃体切割手术对角膜内皮细胞影响的相关因素。 方法 回顾分析 2 13例玻璃体切割手术患者 2 13只眼手术前及手术后 1周、1、3个月时角膜内皮镜检查结果。其中单纯玻璃体切割手术78只眼 ,玻璃体切割手术联合白内障摘除手术 135只眼 ;硅油填充 34只眼 ,C3F8填充 5 3只眼 ,无眼内填充物者 12 6只眼。 结果 单纯玻璃体切割组、玻璃体切割联合白内障摘除后囊完整组手术前后不同时期角膜内皮细胞数差异无显著性的意义 (P均 >0 .0 5 ) ;无后囊且眼内填充 C3F8或硅油组手术后较手术前角膜内皮细胞数明显减少 ,其差异有显著性的意义 (P<0 .0 5 )。 结论 玻璃体切割联合白内障摘除且晶状体后囊不完整者 ,眼内填充 C3F8或硅油对角膜内皮细胞有一定的损伤作用。

Objective To evaluate the relative factors of effect of vitrectomy on corneal endothelial cells. Methods Retrospective analysis of the results of corneal endothelium microscopy performed on 213 eyes of 213 patients undergone vetrectomy operations including single vitrectomy (78 eyes), vitrectomy combined with cataract extraction (135 eyes), silicone oil injection (34 eyes), and C 3F 8 injection (53 eyes) before and after 1 week, 1 and 3 moths of these surgical procedures. Results There was no significant difference between pre- and postoperative corneal endothelium density in single vitrectomy group and vitrectomy combined with cataract extraction with posterior capsule integrity group ( P >0.05). The corneal endothelium density significantly decreased postoperatively in C 3F 8 or silicone oil injection group with broken posterior capsule ( P <0.05). Conclusion C 3F 8 and silicone oil may damnify corneal endothelium in patients undergo vitrectomy combined with cataract extraction with broken posterior capsule.
