河南省大范围白内障复明工作的体会

Experience of large scale sight restoring for cataract blind patients in Henan province

目的 探讨大范围白内障手术复明工作的组织管理和开展复明手术的方法。方法 总结河南省 1988～ 2 0 0 3年建立全省技术网络、有组织地开展人员培训及大范围白内障手术复明工作的经验。随机抽样调查了新的小切口非超声乳化手术效果。结果 15年来各类白内障复明手术累计 411886例。抽样调查 12 5 66例术后 1～ 2天视力。视力≥ 0 .5者占 86.2 5 % ,0 .8～ 1.0者占 80 .44 %。每例手术所用时间大多在 10～ 2 2分钟之间 ,其中娩核时间 8～ 2 6秒。术中后囊破裂 2 .41% ,玻璃体脱出 1.14 %。脱盲率 99.12 % ,脱残率 96.86%。结论 在一个省范围内 ,采取适当措施和适用的手术方法 ,可以提前实现世界卫生组织与防盲协会所提出的“视觉 2 0 2 0”行动纲要的部分目标 ,先使白内障盲人“享有看见的权力”。

Objective To evaluate the organizing and supervision method of large scale sight restoring for cataract blinds and the rehabilitation surgery methods.Methods To summary the sight restoring work in Henan province from 1988 to 2003 retrospectively: setting up technical network in whole province, carring out personnel training organically. Random sampling survey of the clinical effect of an improved small incision non_phacoemulsification ECCE and IOL implantation for cataract patients with preoperative vision ≤0.1.Results 411886 cases of cataract patients had accepted the sight retoring surgery in 15 years. 1 or 2 days after the surgery, 86.25% had a vision of 0.5 in 12566 random sampling cases, and 80.44% had a vision of 0.8-1.0. The surgery time were 10-22 minutes mostly in each cases. Capsular rupture occurred in 2.41% and vitreous prolapse in 1.14%. 99.12% of the cases had been out of blindness (≥0.05) and 96.86% got a bread_and_butter vision (≥0.3).Conclusion Using proper surgical method and with a systemic administration, the purpose “Vision 2020” can be got ahead of schedule in a province.
