摘要
目的探讨前、后交通动脉瘤的内镜辅助显微手术夹闭方法及可能的风险和对策。方法对经过DSA造影证实的12例前交通、后交通动脉瘤病人,在显微镜的引导下置入内镜,观察瘤颈与周边血管、神经的关系,辅助分离瘤颈与周边的黏连;瘤颈夹闭后观察动脉瘤夹与周围结构的关系。结果10例成功夹闭,2例因黏连致密而包裹治疗。结论内镜辅助的显微神经外科是颅内动脉瘤外科治疗的重大进步,可有效显露动脉瘤周边的结构,提高动脉瘤的手术夹闭率。
Objective To discuss the method of the endoscope-assisted clipping of anterior or posterior communication artery aneurysms. Methods In 12 cases of AcoA and PcoA aneurysms, the endoscope ailed with microscope was applied to observe the relationship between the neck and vessel and nerve; and to observe whether the aneurysm has been correctly clipped. Results We correctly clip ten of twelve aneurysms. Conclusion Endoscope-assisted microsurgery is a major advance in the microsurgical treatment of intracranial aneurysms, we can distinctly observe the structure surrounding the aneurysms with it, the clipping rate has been effectively raised.
