吡喃阿霉素膀胱内灌注预防膀胱癌术后复发的临床研究

Intravesical instillation of pirarubicin in the prophylaxis of local recurrence after resection of bladder cancer

目的 探讨吡喃阿霉素 (THP)膀胱内灌注预防膀胱癌术后复发的疗效和安全性。方法 对 43例初发的浅表性膀胱癌术后患者定期进行THP膀胱内灌注治疗 ,随访 12～ 3 6个月。结果 无肿瘤复发率为 88.4% ,复发率为 11.6% ,12例于膀胱灌注期间出现膀胱刺激症状 ,其中 2例因为不能耐受 ,暂停灌注 ,其余患者均完成治疗。所有患者未见明显的全身性药物反应。结论 THP膀胱内灌注预防膀胱癌术后复发疗效满意 ,病人耐受性好 ,无全身性药物反应。

Objective To investigate the effect of intravesical instillation of pirarubicin or mitomycin in the prophylaxis of local recurrence after resection of bladder cancer.Methods Eighty six patients with bladder cancer after TURBt or partial cystectomy were randomly divided into two groups to receive intravesical instillation of pirarubicin or mitomycin respectively.All these patients were followed up for 12 to 36 months.Results Recurrence rate in the patients treated with pirarubicin was 11.6%.No systemic side effect has been observed in all patients.Conclusion Intravesical instillation of pirarubicin has been effective against local recurrence of bladder cancer without systemic side effects.
