急性呼吸窘迫综合征家兔肺部及肺外器官炎症反应的变化 被引量:12

摘要 目的 通过生理盐水肺泡灌洗法复制急性呼吸窘迫综合征 (ARDS)家兔模型 ,研究肺部及肺外器官炎症反应的变化。方法 生理盐水肺泡灌洗法复制ARDS家兔模型。以正常家兔为对照组。ARDS模型复制成功后 ,测定肺组织湿重 /干重比值 (W/D) ,迁移率改变电泳法 (EMSA)测定肺组织核因子 κB(NF κB)活性 ,逆转录PCR(RT PCR)检测组织肿瘤坏死因子 α(TNF α)和白细胞介素 10 (IL 10 )mRNA表达 ,酶联免疫吸附法 (ELISA)测定组织TNF α和IL 10浓度 ,比色法测定组织髓过氧化物酶 (MPO)及丙二醛 (MDA)含量。结果 ARDS家兔肺组织W/D为 5 84± 0 5 4 ,明显高于对照组 ( 4 5 7± 0 36 ,P <0 0 5 ) ;肺组织NF κB活性ARDS组为 2 89 0 5± 10 1 38,与对照组比较显著增高 ( 14 1 89± 33 86 ,P <0 0 5 )。与对照组比较 ,ARDS家兔肺、肝和肠组织TNF α及IL 10mRNA表达均显著增高 (P <0 0 5 ) ,但组织TNF α及IL 10浓度均无明显变化。ARDS家兔肺组织MPO和MDA水平均明显高于对照组 (P <0 0 5 ) ;肝肠组织MPO和MDA含量ARDS组与对照组比较无明显变化。结论 生理盐水肺泡灌洗法复制的ARDS家兔模型 ,通过活化NF κB ,导致肺部炎症反应增强 ,同时可激活肺外器官的炎症反应。 Objective To investigate the pulmonary and extrapulmonary organs inflammatory response in acute respiratory distress syndrome (ARDS) rabbit model made by saline alveolar lavaged Methods Twelve New Zealand rabbits were randomly assigned to two groups (control group and ARDS group) Lung wet/dry weight ratios (W/D) were recorded to assess lung injury Lung homogenates were prepared and NF κB activity was detected by electrophoretic mobility gel shift assay (EMSA) while tumor necrosis factor α (TNF α) and interleukin 10 (IL 10) levels were detected by enzyme linked immunosorbent assay (ELISA) and their mRNA expression by reverse transcription polymerase chain reaction (RT PCR) Myeloperoxidase (MPO) and malondialdehyde (MDA) in lung homogenates were assessed too Liver and intestine homogenates were assayed for MPO and MDA while the concentration of TNF? and IL 10 by ELISA and their mRNA expression by RT PCR Resultes In ARDS group,W/D (5 84±0 54) was higher than that in control group (4 57±0 36) significantly ( P <0 05) As compared with the NF κB activity in control group (141 89±33 86),NF κB activity in ARDS group increased significantly (289 05±101 38) As compared with control group,mRNA expression of TNF α and IL 10 increased significantly in the lung,liver and intestine homogenates of ARDS group In ARDS group,the concentration of MPO and MDA in lung homogenates were higher than that in control group significantly,but no significant difference was found in liver and intestine homogenates The concentration of TNF α and IL 10 in control group were not different from those in control group Conclusions Saline alveolar lavage could activate NF κB and up regulate inflammatory mediator expression in ARDS rabbit mennwhile trigger the inflammatory response in the extra pulmonary organs
