老年性黄斑变性合并白内障的手术疗效分析

The clinical analysis of cataract surgery of patients with age-related macular degeration

目的 探讨伴有较严重老年性黄斑变性的白内障患者的手术治疗效果 ,评价其手术价值。方法 回顾整理伴有较严重AMD患者的白内障手术治疗 ,共 3 5例 3 8只眼 ,对其中 2 7名患者的2 9只眼实施的是超声乳化人工晶体植入术 ,对其余 8名患者的 9只眼实施的传统白内障摘除、人工晶体植入术。结果 术后患者视力除 1例术后视力不提高外 ,其他均较术前有不同程度的提高 ,其中指数 2只眼 ( 5 3 % ) ,0 0 1～ 0 0 8者 11只眼 ( 2 8 9% ) ,0 1～ 0 2者 12只眼 ( 3 1 6% ) ,0 3者 8只眼( 2 1 1% ) ,0 4～ 0 5者 5只眼 ( 13 2 % ) ;术后黄斑病变没有进一步加重。结论 较严重的AMD的患者如果同时伴有成熟的白内障 ,在白内障严重影响视力的情况下 。

Objective To analysis the effects of cataract surgery of patients with moderate and severe age related macular degeneration(AMD),and asses the value of the surgery.Methods 38 Eyes of 35 patients with moderate and severe AMD who had accepted cataract surgery are reviewed.Phacoemusification was performed in 29 eyes of 27 cases and ECCE+IOL was performed in 9 eyes of 8 cses.Results All patients had an improvement in best visual acuity except 1 patient had not improvement.2 eyes(5 3%)got CF,11 eyes(28 9%) got 0 01～0 08,12 eyes(31 6%) got 0 1～0 2,8 eyes(21 1%) got 0 3,5 eyes(13 2%)got 0 4～0 5;All patients did not feel worse than preoperatively.Conclusions Patients with moderate and severe AMD and mature cataract can get benefit from cataract surgery.Cataract surgery is necessary when the visual acuity is very low.
