肺血少型复杂紫绀型先天性心脏病手术的护理

Nursing about pulmonary ischemic complicated cyanosis congenital heart disease operation

目的 :探讨肺血少型复杂紫绀型先天性心脏病手术的护理措施。方法 :做好术前准备 ,呼吸机参数调节适宜 ,尽早拔管 ,采用“V”型体位 ,合适的中心静脉压 ,以维持循环稳定及预防并发症。结果 :全组 37例死亡 3例 ,余 34例并发乳糜胸、肺不张各 1例 ,胸腔积液 7例 ,均治愈出院。随访1～ 38个月 ,术后患儿活动耐力明显提高 ,平均动脉血氧饱和度由 (73± 7) %提高至 (92± 5 ) %。结论 :做好术前护理 ,术后取正确体位 ,早期拔管 ,强化呼吸道护理 ,维持循环稳定 ,及早抗凝 ,是肺血少型复杂紫绀型先天性心脏病手术后康复的正确措施。

Objective:To explore nursing measures about pulmonary ischemic complicated cyanosis congenital heart disease patients.Method:We had prepared for operation,adjust for favourable respiratory machine index,remove tube as soon as possible,accepted “V”shepe posture.Control suitable central veinous pressure.Prevented from complications.Result:3 died,other 1 case complicated with chylothorax and atelectasis,7 cases pleura infusion,all healed and discharged.Blood oxygen saturation raised from (73±7)% to (92±5)% with 1～38 months follow up.Conclusion:Good preparation suitable posture,early remove of tube,management of respiratory tract,stable circulation anticoagulation can ensure successful operation.
