肾盂成形术后肾切除9例分析

A clinical analysis of 9 cases of nephroectomy after Anderson-Hynes pyeloplasty

目的 探讨肾盂成形术后肾切除的原因及预防对策。方法 回顾性分析 9例肾盂成形术后肾切除的临床资料。结果 9例患者因发热、腰痛、肾区包块、脓尿、肾造瘘管无法拔除于肾盂成形术后 2月～ 3年行肾切除术。结论 手术方式选择不当、再梗阻、感染是导致手术失败肾切除的原因 ,其中感染是最关键的因素。提高术者的手术操作技能 。

Objective To investigate the etiology and preventive strategy of nephrectomy after Anderson-Hynes pyeloplasty for ureteropelvic junction obstruction (UPJO). Methods A clinical data of 9 cases nephroectomy after Anderson-Hynes pyeloplasty for UPJO were analyzed retrospectively. Results Nephroectomy was performed in 2 months to 3 years after Anderson-Hynes pyeloplasty due to fever, loin pain and lump, pyuria and failure to remove the nephrostomy tube. Conclusion The chief etiology of nephroectomy after Anderson-Hynes pyeloplasty for UPJO was improper operation pattern, recurrent obstruction and infection, and most important of all infection. The kidney loss can be avoided by improve the operators skill and correct perioperative management.
