掌黑癣1例

A case of tinea manuum nigra

报告1例掌黑癣。患者男,64岁。右手大鱼际出现黑色斑疹4年,无自觉症状,夏季发作,冬季可自愈。真菌镜检见棕色分隔菌丝,真菌培养见黑色酵母样菌落,镜下为棕色分隔菌丝,大量厚壁孢子。用5%水杨酸乙醇和硝酸咪康唑(达克宁)乳膏治疗2周后皮疹消退。

A 64-year-old male patient presented dark macules on his right palm for about 4 years without significant symptoms. The lesion reoccurred in summer and disappeared in winter. The direct microscopic examination of the lesion showed the brownish septate hyphae. The fungus grew on the medium into a black yeast-like colony with brown septate hyphae and lots of chlamydospores under the microscope, which was then identified as Hortaea werneckii. The lesions were gradually vanished by topical administration of 5% tincture of salicylate and 1% miconazole cream alternatively.
