摘要
目的 探讨脉冲波多普勒组织成像技术 (PW- DTI)定量评价慢性肺心病患者右室功能的临床价值。方法 对 5 0例慢性肺心病患者和 30例正常人进行经胸超声检查 ,采用心尖四腔心切面 ,应用 PW- DTI对右室侧壁、室间隔、三尖瓣前叶及隔叶瓣环等部位的运动进行定量分析。结果 与对照组相比 ,早期慢性肺心病患者 PW- DTI各参数无明显改变 ;晚期慢性肺心病缓解期患者各部位收缩期峰值运动速度 (Vs)、舒张早期运动速度 (VE)明显减低 ;晚期慢性肺心病急性发作期患者除 VS、VE进一步减低外 ,舒张早期运动速度与舒张晚期运动速度之比 (VE/ VA)亦显著降低。结论 DTI能准确、实时地反映右心功能 。
Objective To evaluate the clinical value of Doppler tissue imaging(DTI) in detecting right ventricular function in chronic pulmonary heart disease. Methods Forty patients with chronic pulmonary heart disease and thirty normal controls were studied with pulse wave DTI (PW DTI).PW DTI indices of right ventricle,interventricular septum and tricuspid valve annulus were detected,then compared with those of the control group.The indices included systolic peak velocity(Vs),early diastolic peak velocity(V E)、ratio of V E and late diastolic peak velocity(V E/V A). Results Compared with normal subjects,V s and V E were not significantly increased in earlier chronic pulmonary heart disease;In late remittent chronic pulmonary heart disease,V S and V E were decreased significantly;In late paroxysmal chronic pulmonary heart disease,V S,V E and V E/V A were all decreased significantly. Conclusions DTI can exactly,real time and quantitatively assess the dynamic changes of myocardiac motion.It provides new reliable indicators for estimating right ventricular function.
