冠脉造影与两种导联心电图T波变化的分析

Preliminary Evaluation of Coronary Angiography and T Wave Change of Routine Leads and Head-chest Leads

目的 探讨头胸导联心电图 (HCECG)和常规导联心电图 (RLECG)与冠状动脉病变的关系。方法 分析 172例两种导联心电图的T波形态 ,并结合其中 15例经冠状动脉造影、心血池 (99mTC RBC)、Holter、UCG、X线心脏片、血脂检查做比较。结果 ①正常组两种导联心电图T波形态一致 ,均在正常范围 ,但T波振幅均值HCECG >RLECG(P <0 .0 1)。②异常组HCECG与对照的几项检查有较好的对应关系。且HCECG能排除T波的假性改变 ,更符合冠脉造影诊断 ,可用来客观反映冠状动脉的病变情况。③HCECG与冠脉造影结果有很好的相关性。结论 HCECG为无创性检查 ,能提供心肌缺血的初步判断 ,有助于冠脉造影前的临床筛选。

Objective To study relationship between CHD and T wave change of the head-chest leads(HCECG) and routine leads (RLECG).Methods T wave was analyzed in 172cases and the two types electrocardiogram was compared with the examinations of the coronary angiography、heart blood pool (99 mTC-RBC)、Holter、UCG、heart X-ray and blood lipids in 15 cases of the whole patients.Results ①In healthy subjects,the T wave of the two electrocardiograms was within normal range,but the amplitude in HCEGC was higher than that in RLECG(P<0.01).②In the patients,the HCECG had a better relationship with the examinations in control cases and owing to its advantage of avoiding the misleading changes,was in consistency with the diagnosis of coronary angiography.③HCECG and the results of coronary aniography have good corelation.Conclusion HCECG,a type of un-injuring examination,may facilitate the preliminary evaluation of myocardial ischaemia before coronary angiography.
