经皮左心耳封堵术的研究进展 被引量：2

摘要 房颤的发生率在一般人群中为0.4%～2.0%,年龄每增加10岁,发病率增加1.4倍[1].研究表明,房颤是发生中风的独立危险因素,15%以上的脑卒中由房颤引起,非瓣膜病慢性房颤病人中脑栓塞发病率是正常人的5倍,而在瓣膜病慢性房颤病人中脑栓塞发病率达到正常人的17倍[2].80岁以上合并房颤脑卒中的患者达到25%,每1000人每年发生40次脑卒中[3].新近结束的AFFIRM[4]和RACE[5]结果显示,维持窦性心律和控制心室率比较,在生存率方面没有优势;控制心室率控制可以减少抗心律失常药的副作用,且费用较低.由于目前房颤的射频消融治疗尚未完全成熟,控制心室率简便易行,疗效至少等同于较复杂的复律和维持窦性心律.这无疑会引导我们针对房颤的治疗策略发生转变.而对心房颤动复律后复发的高危病人,尤其应首先考虑选择控制心室率.因此预防房颤的栓塞并发症显得尤为重要.
