140例小儿癫癎与可疑癫癎的24h动态脑电图监测报告

Analysis of AEEG monitoring in 140 children with clinically-diagnosed epilepsy or suspected epilepsy

目的 :探讨小儿动态脑电图 (AEEG)的睡眠时相与癫的关系及其对小儿癫病的诊断价值。方法 :14 0例常规脑电图 (EEG)无样放电患儿分别为临床诊断癫组和可疑癫组 ,进行 2 4hAEEG监测 ,并观察觉醒期和睡眠期AEEG样放电的分布情况。结果 :AEEG样放电发生率在临床诊断小儿癫组占 5 5 9% ,小儿可疑癫组占 37 5 %。样放电多见于NREMⅠ Ⅱ期 ,并见有NREMⅠ Ⅱ期明显延长 ,而NREMⅢ Ⅳ期相对缩短。结论 :小儿癫病样放电主要发生于NREMⅠ Ⅱ期 ,AEEG可提高小儿癫病样放电的检出率 。

Objective:To explore the relation of children AEEG between sleeping phase and epilepsy, and its diagnosis value for children epilepsy. Methods:AEEG was performed on 140 patients with no epileptiform discharges (divided into clinical diagnosed epilepsy group and suspected epilepsy group). Distribution of AEEG epileptiform discharge was observed during waking up and sleeping.Results:The positive rate of epileptiform discharge in children with clinically diagnosed epilepsy was 56%, and 37.5% in the suspected epilepsy. The epileptiform discharges were mainly found during NREM Ⅰ Ⅱ. It was observed that NREM Ⅰ Ⅱ was lengthened and NREM Ⅲ Ⅳ was shortened. Conclusions:Children epileptiform discharges happeared mainly during Ⅰ Ⅱ. AEEG might increase the detection rate of epileptiform discharges. It might be of important significance in the diagnosis of epilepsy.
