超声在早期卵巢癌诊断中的价值

Diagnostic value of ultrasound in early ovarian carcinoma

目的 探讨超声在早期卵巢癌诊断中的价值。方法 分析经超声、肿瘤标记物CA12 5检查 ,并经手术及病理证实的早期卵巢癌患者 47例。结果 肿瘤呈囊性 2 9例 ,内有乳头或实性成分 ,单房 5例 ,多房 2 4例 ;囊实性 13例 ;实性 5例。超声发现卵巢病变 46例 ,诊断卵巢恶性病变 42例。术前超声诊断的敏感性、特异性、阳性预测值和阴性预测值分别为 89.4%、92 .7%、43 .8%和 99.3 %。 47例早期卵巢癌患者中 ,CA12 5 >3 5U /ml者 2 4例 ,敏感性、特异性、阳性预测值和阴性预测值分别为 5 1.1%、90 .2 %、2 5 .0 %和 96.7%。超声对早期卵巢癌的敏感性较血清CA12 5高 (P <0 .0 1)。结论 超声在早期卵巢癌诊断中起着重要的作用 ,为早期卵巢癌的首选检查方法。

Objective To evaluate the value of ultrasound in diagnosis of early ovarian carcinoma. Methods The ultrasound imaging,tumor marker CA125 in 47 cases with early ovarian carcinoma were analyzed. All the cases were confirmed by pathology. Results Twenty-nine patients showed cystic tumor with papilla or solid projection,of which 5 were unilocular,24 were multicolular;thirteen patients showed solid tumor with cystic component inside;five patients showed solid tumor. In 46 cases ovarian neoplasms were demonstrated by ultrasound,ovarian malignancies were diagnosed in 42 cases,sensibility,specificity,positive predictive value and negative predictive value of ultrasound diagnosis before operation were 89.4 %, 92.7 %, 43.8 % and 99.3 % respectively. In 47 cases,serum CA125>35 U/ml in 24 cases. Sensibility,specificity,positive predictive value and negative predictive value were respectively 51.1 %, 90.2 %,25% and 96.7 %. Sensibility in early ovarian carcinoma by ultrasound was higher than that by serum CA125 (P< 0.01 ). Conclusions Ultrasound plays an important role in diagnosis of early ovarian carcinoma. It is the first method to detect early ovarian carcinoma.
