摘要
目 的 :探讨HBsAg、HBeAg阳性孕妇外周血单个核细胞 (PBMC)内乙型肝炎病毒(HBV)DNA感染状况及其在宫内母婴垂直传播中的作用。方法 :对HBsAg/HBeAg双阳性共6 7对孕妇及其新生儿静脉血分离和提纯PBMC后 ,经抽提、纯化后的DNA进入PCR扩增反应 ,引物为HBVC区基因序列。结果 :6 7名HBsAg及HBeAg双阳性的孕妇中有 35例 ( 5 2 .2 % )PBMC中HBVDNA阳性 ,2 5例孕妇在血清及PBMC中均发现HBVDNA。 6 7名新生儿有 2 2例感染HBVDNA ,感染率 32 .8% ,其中血清HBVDNA阳性者 10例 ,PBMCHBVDNA阳性者 19例 ,二者均阳性者 7例。结论 :母亲PBMC内HBVDNA阳性可能导致新生儿PBMC中HBVDNA阳性 ,PBMC内的HBVDNA可能是HBV母婴垂直传播的一条重要途径 ,同时 。
Objective:The purpose of this study was to determine the HBV infection rate of PBMC in pregnant women with both postive HBsAg and HBeAg and their newborn and to evaluate the role of infected PBMC in the intrauterine. Methods: 67 mothers- infants pairs participated in the study, PBMC were isolated from the same blood sample. The DNA was extracted and purified, then DNA samples were set for the PCR procedure using the primers specific for the c gene sequence. Results:35(52.2%) of the 67 positive HBsAg and HBeAg pregnant women had been detected PBMC HBV DNA and serum HBV DNA, respectively. 25 of whom were all detected HBV specific DNA in PBMC and plasma samples.22(32.8%)of 67 newborns infected HBV, including 10 newborns with serum HBV DNA and 19 newborns with PBMC HBV DNA, 7 newborns were all detected HBV DNA in PBMC and plasma sample. Conclusion:That infection with HBV in maternal PBMC is responsible for perinatal newborn's PBMC HBV infection and it may be an important route of HBV vertical transmission. Infants whose mothers had sera positive for both HBsAg and HBeAg and HBV DNA had extraordinarily high risk of hepatitis B virus infection.
