血清TNF-α及h-EGF含量与重型病毒性肝炎的预后

The relationship of serum TNF-α and h-EGF levels and prognosis of severe viral hepatitis

目的 为探讨重症肝炎预后的影响因素 ,本文检测了 4 9例重肝患者血清 TNF-α和 h-EGF含量变化。方法 采用 ABC- EL ISA方法分别测定血清 TNF-α和 h- EGF含量。结果 死亡组患者血清 TNF-α水平治疗前稍高于存活组 ,且与对照组比较差异有显著性 ,而 h- EGF却相反。随着病情进展 ,死亡组 TNF-α水平升高 ,h- EGF水平降低 ;存活组与之相反。血清 TNF-α水平越高 ,病死率越高 ;而 h- EGF水平越高 ,病死率越低。结论 TNF-α及 h- EGF分别能反映肝细胞坏死及再生情况 ,其动态检测能较好预测重肝发展趋势 。

Objective To study the influencing factors the prognosis of severe hepatitis, we determined the serum levels of TNF-α and human epidermal growth factor (h-EGF) in 49 patients with severe hepatitis. Methods ABC-ELISA was used for testing the serum levels of TNF-α, h-EGF respectively. Results Serum TNF-α levels of death group before therapy were lightly higher that of survival group, P value of were <0.01 while comparing with control group, but the level of h-EGF was just opposite. AS liver disease progressed gradually, serum TNF-α levels of death group increased while h-EGF level decreased. But those of survival group were opposite. The higher level serum TNF-α were, the higher mortalities were; but the higher level serum h-EGF were, the lower mortalities were. Conclusions Serum TNF-α and h-EGF can indicate the degree of hepatic necrosis and regeneration respectively. The dynamic determination of them might predict fairly the trend of severe hepatitis prognosis, and were used as indices of evaluating severe hepatitis prognosis.
