摘要 背景:高强度超声(HighIntensityUltrasound,HIU)在治疗临近大神经的器官、组织时,提供神经保护相关的实验依据及超声在周围神经领域的应用(如超声阻滞疗法)的实验基础及依据研究均较少。目的:观察HIU作用于兔坐骨神经干后的病理变化及其剂量与预后的相关性。设计:完全随机设计。地点和材料:实验在重庆医科大学医学超声工程研究所完成,以96只新西兰大白兔为研究材料。干预:以不同的HIU剂量(强度×时间)辐照兔坐骨神经干,观察辐照处各时间点神经组织形态变化。主要观察指标:辐照处各时间点神经组织形态变化。结果:大体观察:辐照后即刻辐照处神经及周围组织无明显变化,25s后,轻微发白,50s后,发白明显;组织学观察:辐照后坐骨神经出现轻微脱髓鞘至坏死改变。结论:坐骨神经受损伤程度与剂量大小有关,剂量小损伤轻,剂量大损伤重。在一定范围内,受损神经可以完全恢复,剂量过大受损神经难以恢复。 BACKGROUND:High intensity ultrasound(HIU) provides experimental basis for protecting the nerves adjacent to the organs and tissues.Little research has been conducted on the experimental basis and evidence of HIU applied in diseases of the peripheral nerves. OBJECTIVE:To observe the pathological changes of rabbit sciatic nerves after exposure to HIU and determine the correlation of HIU dosage with its prognosis. DESIGN:A completely randomized design was performed. SETTING and MATERIALS:The study was carried out in the Ultrasonic Research Institute of Chongqing University of Medicine.Ninety six New Zealand rabbits were randomly divided into 4 groups with 24 in each. INTERVENTIONS:The rabbit sciatic nerves were exposed to HIU of different doses. MAIN OUTCOME MEASURES:The histological changes of the nerves exposed to HIU were observed at different points of time. RESULTS:There were no significant changes in the nerves irradiated and tissues around between 0 s and 15 s groups;they slightly whitened in the irradiated area in 25 s group and significantly whitened in 50 s group.The histological observation revealed slight demyelination or even necrosis. CONCLUSION:The degree of sciatic nerve injury is associated with HIU dosage.The larger the dosage is,the more severe the injury is,and vice versa.The higher injury degree,the more difficult the nerves are repaired although the injured nerves can be fully recovered within a certain range of dosage.
