老年结肠癌并多脏器病变患者的围手术期处理

PERIOPERATIVE MANAGEMENT FOR THE AGED PATIENTS WITH CARCINOMA OF COLON AND MULTI-ORGAN DISEASES

目的 探讨老年结肠癌并多脏器病变患者围手术期的处理。方法 回顾性分析 81例老年结肠癌并多脏器病变患者的临床资料 ,术前作相应的内科治疗和充分的肠道准备 ,术中施行恰当安全的手术方式 ,术后加强监护和营养支持治疗 ,积极防治并发症。结果 81例中手术切除率为 97 5 % ,根治性切除占74 0 % ,姑息性切除占 2 3 5 % ;术后并发症发生率 30 8% ,围手术期死亡率 3 7%。结论 只有重视老年结肠癌并多脏器病变患者围手术期的处理。才能提高本病的手术疗效。

Objective To summarize the perioperative management for the aged patients with carcinoma of colon and multi-organ diseases. Methods Clinical data of 81 cases were analyzed retrospectively, the measures recorded including that of making medical therapy and enough intestinal preparation before operation, performing appropriate and safe operating ways, strengthening care and nutrition support after operation, preventing and curing the complications actively. Results The rate of resection was 97.5%, radical resection was 74.0%, palliative resection was 23.5%;postoperative complication was 30.8%; death rate was 3.7% . Conclusion Pay attention to the perioperative management for the aged patients with carcinoma of colon and multi-organ diseases, just increased the curative effect.
