脊髓纵裂合并脊柱侧弯的手术处理

Surgical treatment of split cord malformation(SCM) complicated with scoliosis

目的 探讨脊髓纵裂合并脊柱侧弯的手术适应证及手术方法的改进措施。方法 总结 2 4例脊髓纵裂合并脊柱侧弯的手术选择和手术方法。结果 2 4例脊髓纵裂骨嵴切除脊膜囊成形手术均获成功 ,继而行脊柱侧弯矫形未发生神经系统并发症。结论 脊柱侧弯畸形患者在做脊柱矫形之前应检查是否合并脊髓纵裂 ,合并脊髓纵裂者应先行椎管内骨嵴切除。

Objective To investigate the operative indication of SCM accompanying with scoliosis and to improve the surgical techniques.Methods The operation and surgical techniques were reviewed retrospectively in 24 cases.Results The septum resection and spinal meninges reconstruction were all successful in these 24 cases.The subsequent scoliosis orthopedics had no complications.Conclusions The patient with scoliosis should be identified whether complicated with SCM or not before the spinal orthopedics is determined.The septum resection and dura mater spinalis plastic repair should be done first in the patients complicated with SCM.The surgical methods demonstrated in this article are safe and effective.
